1. WHAT IS TELEHEALTH?
Telehealth involves the use of electronic communications, including video conferencing, telephone, and secure messaging, to enable OnPoint Healthcare Services LLC providers to connect with you for healthcare services. This may include evaluation, diagnosis, consultation, treatment, follow-up, and health education, all conducted remotely using secure technology.
2. HOW YOUR TELEHEALTH VISIT WORKS
Your visit will take place via a HIPAA-compliant video platform or secure phone call.
You will need a device with internet access (smartphone, tablet, or computer) and a private, quiet location.
Your provider is Marlene Margerum, DNP, AGPCNP-C, CRNP, licensed in Maryland.
Technical difficulties may occasionally interrupt or delay your visit. Staff will contact you to reschedule if needed.
Prescriptions, lab orders, and referrals may be issued electronically following your visit.
3. BENEFITS OF TELEHEALTH
Access care from the comfort of your home or any private location.
Reduced travel time, cost, and barriers to care.
Timely access to your provider, especially for follow-ups and chronic disease management.
Continuity of care for established patients unable to visit in person.
4. RISKS & LIMITATIONS
Information transmitted electronically may, in rare cases, be subject to interception or security risks despite all reasonable safeguards.
Telehealth may not be appropriate for all conditions. Your provider will determine if in-person care is necessary.
Physical examination is limited in a telehealth setting; some diagnoses may require in-person follow-up.
Technical issues (internet disruptions, audio/video problems) may affect quality of care.
Emergencies cannot be managed via telehealth — you must call 911 or go to the nearest emergency room.
5. EMERGENCY PROCEDURES
IF YOU ARE EXPERIENCING A MEDICAL EMERGENCY, CALL 911 IMMEDIATELY. Do not use telehealth services for emergencies. Your provider will refer you to in-person emergency services when appropriate.
6. CONFIDENTIALITY & PRIVACY
All telehealth sessions are conducted using HIPAA-compliant, encrypted platforms.
Sessions are NOT recorded without explicit written consent from the patient.
You are responsible for ensuring the privacy of your physical location during visits.
Do not conduct your telehealth visit from a public location or shared device.
Your health information is protected under HIPAA and our Notice of Privacy Practices.
7. TECHNOLOGY REQUIREMENTS
A working smartphone, tablet, or computer with camera and microphone.
A stable internet connection (Wi-Fi or cellular data).
A private, well-lit space where you can speak freely.
You will receive a secure link or call-in number prior to your appointment.
8. OUT-OF-STATE PATIENTS
OnPoint Healthcare Services LLC is licensed to provide care to patients located in Maryland at the time of the telehealth visit. If you are located outside of Maryland during your visit, please notify your provider prior to the appointment, as additional licensure requirements may apply.
9. PATIENT RIGHTS
You have the right to refuse telehealth services at any time without affecting your right to future care.
You may request an in-person alternative when clinically appropriate.
You have the right to request copies of all records from your telehealth visits.
You may withdraw this consent at any time by notifying your provider in writing.
